The pages of our Catalog have order buttons available in the right-hand column.  You can use them to order your _TM online, directly from the catalog, via PayPal, using a credit card.  See our Info page for more details.  

This page provides access to a printable order form.  The form is for those who wish to order by mail and pay using a check or money order.  It should be self-explanatory.  If you don't find it so, please feel free to call us at 412 818-4044, or send us an email at with your questions -- we'll either have answers or get them for you.

Click on the image to the left to access a printable order form in PDF/Adobe Acrobat Reader format (about a 30 sec download at 56.6). When the form appears, check your printer to be sure it's turned on and has paper loaded, then click on the printer icon in the Adobe Reader toolbar.  For best results, set your printer parameters to print in "Landscape" mode.
